Membership includes access to Colleen Lizewski, PA-C for services described above. When needed , lab requisitions will be sent to Lab Corp. and your insurance coverage will be used subject to your normal co-pays and deductibles.

Do I need to keep my health insurance?
Yes. You will maintain your health insurance as a member of our practice. We are not credentialed with commercial insurance, Medicare or Medicaid.
